Insurance

Lake Five Dentistry is a preferred provider for many insurance companies, and we work with most traditional insurance carriers. We do not, however, work with Health Management Organizations. Our staff can verify your insurance eligibility before your first appointment and determine your out-of-pocket costs before treatment.

We are also happy to file your insurance for you. Any fees not covered by insurance are due at the time of service.
